Initial Consultation Information

REFERRAL

In order to see Dr Pascoe for an initial consultation, you will require a referral from your GP or specialist.  GP referrals last 12 months, with specialist referrals lasting 3 months.

Please have your referring practitioner fax or electronically send a copy of your referral, along with any other relevant tests or scans that you may have had, through to us directly.  On receipt of your referral information we will contact you to arrange an appointment.  Having this information in front of us when arranging your appointment ensures that we book you in for the correct consultation +/- any tests that Dr Pascoe may feel you need to have on the day, at our practice, before seeing her.

You should allow between 20 min and half an hour for your initial consultation.

On booking your appointment we will send you an email with your appointment details, our location, cost and other relevant information.

PARKING

There is free on street parking nearby.  We suggest parking on Sanders street, on the lower level car park of Coles at the end of Sanders street or at Garden City Shopping Centre, a short 5-10min walk away.

PRIOR TO YOUR APPOINTMENT

Please ensure we have a copy of your referral addressed to Dr MELINDA PASCOE before you arrive. FAX: 07 3532 0272 or email pascoe.neurology@gmail.com.

PLEASE CONFIRM YOUR APPOINTMENT TWO DAYS PRIOR WITH OUR OFFICE.  This can be done by email or phone, we appreciate your assistance.

A cancellation fee may be charged if less than 24 hours notice is given that you can not attend your appointment, or you simply do not show up.  

​

Complete and bring with you your new patient forms.

​

PAYMENT

Payment is required in full on the day, we are not a bulk billing practice. Initial Fee is $450 (pensioners $375) - Medicare rebate $146.45).  We accept credit card, eftpos and cash. We will send your claim to Medicare for processing. Medicare will then pay any applicable rebates to your nominated bank account.
