Neurology Solutions, under the lead consultancy of Dr Melinda Pascoe, respects your privacy and handles personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles and other applicable health-record requirements. This policy explains how we collect, hold, use, disclose and protect personal information, including information submitted through the NeuroTracker Clinical mobile application.

 

## Information we collect

We collect information reasonably necessary to provide and administer health services. Depending on your dealings with us, this may include: - identity and contact information, including your name, date of birth, address, telephone number and email address; - referral, appointment, billing and Medicare information; - health information, clinical history, investigation results, correspondence and treatment information; - information you provide through clinic forms, telephone calls, email or our website; and - information submitted through NeuroTracker Clinical, including your name, selected neurological condition, chosen symptoms, symptom scores, wellness score, check-in date and time, and app-generated identifiers used to associate entries with your profile and prevent duplicate submissions. Health information is sensitive information. We generally collect it directly from you, your authorised representative, a referring practitioner, another healthcare provider involved in your care, or through NeuroTracker Clinical with your consent.

 

## How we use information

We may use personal information to: - provide, coordinate and monitor your clinical care; - communicate with you and healthcare providers involved in your care; - manage referrals, appointments, accounts and health records; - review symptom and wellness information submitted through NeuroTracker Clinical; - maintain, secure and troubleshoot our clinical and information systems; - meet legal, professional, insurance and record-keeping obligations; and - respond to enquiries, access or correction requests, and complaints. NeuroTracker Clinical information is not sold and is not used for advertising or marketing. ## Disclosure of information We may disclose relevant personal information where reasonably necessary for your care or practice administration, including to healthcare practitioners and services involved in your care, authorised representatives, Medicare or insurers, and contracted service providers that support our secure clinical, communications or information systems. We may also disclose information where you have consented, where disclosure is required or authorised by law, or where necessary to prevent a serious threat to life, health or safety. We do not disclose health information for direct marketing without specific consent. ## NeuroTracker Clinical storage and transmission Your NeuroTracker Clinical profile and check-in history are stored locally on your device. Check-ins are transmitted over an encrypted connection to a clinic-controlled system. If transmission fails, the app retains the pending check-in on your device and retries later. Resetting or uninstalling the app may remove information stored locally on that device. It does not delete information already received and retained by the clinic as part of its health records. NeuroTracker Clinical is not a medical device and does not diagnose, treat, cure or prevent any medical condition. It is not continuously monitored and must not be used for urgent or emergency assistance. In an emergency, call 000.

 

## Security and retention

We take reasonable technical and organisational steps to protect personal information from misuse, interference, loss, and unauthorised access, modification or disclosure. These measures include access controls, passwords, firewalls, encryption, secure backups and limiting access to authorised people. No electronic system is completely risk-free. Please protect your device with a passcode, install security updates and notify us promptly if a device containing NeuroTracker Clinical information is lost or compromised. We retain health information for the periods required by applicable laws and professional obligations. When information is no longer required and we are not legally required to retain it, we take reasonable steps to destroy or de-identify it securely.

 

## Access and correction

You may request access to personal information we hold about you or ask us to correct information that is inaccurate, out of date, incomplete, irrelevant or misleading. We may ask you to make the request in writing and verify your identity. Legal exceptions may apply, and a reasonable access fee may sometimes be charged where permitted. There is no fee for requesting a correction.

We will acknowledge and investigate privacy complaints and aim to respond within a reasonable period. ## Changes to this policy

We may update this policy when our practices, technology or legal obligations change. The current version and effective date will remain available on this website.
